You may find the following suggested strategies helpful in terms of navigating and developing healthy relationships within the health care system.  If you have not already done so, create a personal binder to keep records of each appointment, diagnostic test and outcome, by discipline of medicine and date order, keeping it up to date.  This information can prove to be helpful for front line care-givers when the need arises.

  • With your family doctor, go over past specialist appointments and diagnostic tests and ask that he/she provide you with a copy of reports for your personal file/records.
  • Discuss the Client Plan of Care Diagram, shown below, with your family physician with the suggestion/request that any specialist appointments be made at one major center to foster an interdisciplinary approach to care.
  • If applicable, ask him/her if there is an Allied Health Network that can assist you in any day to day life details. ie: insurance form completion, work and home life challenges and details.
  • Provide your family doctor and any specialists, contact information for the ILC Charitable Foundation should they wish to reach out for Medical Professional-to-Medical Professional Peer Support through the ILC network.
